Foster Care Manager/Therapeutic Case Manager
The Foster Care Case Manager/Therapeutic Case Manager position ensures the safety, performance, and well-being of children in out-of-home foster care. This occurs through the development of individual goal plans to ensures the provision of quality service delivery.
Requirements: Bachelor's degree in Human Services, Social Work, or related field or related experience. Experience working with special needs/at-risk children and families required.
In-Home Crisis Worker
The In-Home Worker will provide intensive home-based family therapy and support services to families whose children and/or adolescents are referred by the Office of Children Youth and Families and are at risk for out-of-home placement.
Requirements: Bachelor's or Associate's degree in Counseling, Social Work, Psychology or related field or at least 2 years of experience employed in social services.
Foster Care Case Aid
The Foster Care Case Aid is responsible for the safety and timely transportation of children in Foster Care to scheduled visits with both parents, siblings, or relatives as assigned by the program coordinator.
Requirements: A high school diploma (or GED) and one-year related experience. Must have a valid driver's license and proof of insurance. Must be at least 21 years old.
Home Health Aide
Follows the plan of care to help the consumer to maintain good personal hygiene and maintain a healthy, safe environment. Home Health Aide is to perform ONLY those functions specified for each individual consumer.
Either 1. High school diploma or equivalent, obtained a valid nurses license in PA; or 2. Demonstrated competency by passing a competency exam developed by the home care agency which meets PA state regulation; or 3. Completed one of the following: a. A training program developed by a home care agency, home care registry, or the other entity which meets the requirements of PA regulation for training; b. A home health aide training program meeting the requirements of 42 CFR 484.36 (relating to the conditions of participation; home health aide services); or c. The nurse aide certification and training program sponsored by the PA Department of Education and located at www.pde.state.pa.us
The program coordinator oversees the daily coordination of the after-school program and summer programming at the GVCS site, including daily upkeep of the attendance database system, monitoring program flow and structure, maintaining school-based relationships with administrators and teachers, and, for the supervision and coordination of program and staff. The coordinator is directly responsible for planning and organizing all daily activities, marketing, and advertisements of programs.
